Requirements for a marriage license issued by Phelps County1. Both applicants must be at least 17 years of age. You cannot marry in Nebraska if you are under 17.2. Parental consent is required for applicants who are 17 or 18 years of age. The parental consent forms are available at the County Clerk's office. 3. One form of photo identification is required -i.e. Drivers license or Passport. 4. You must provide the birthplace of bride and groom. 5. You must provided the full names and birthplaces of both parents. 6. You must provide the social security numbers of bride and groom. 7. If the applicant has had a previous marriage, we will need to know the date the marriage ended. (Date of divorce decree or date of death) 8. No blood test is required. 9. Licenses are good for one year from date of issuance. Therefore, the bride and groom have to be married within that year or they must apply again. 10. A couple may apply for their license in any county in Nebraska and use it to be married in any other Nebraska county.
